		CatalystTM CatalystTM is a PHS-developed, point-of-care clinical information system that provides access to clinical data, ensures follow-up of required services to patients and delivers the application by utilizing technology that is feasible to install and support. 	Chronic Care Tracking A core competency of PHS is a focus on management of high-prevalence, high-cost medical conditions such as diabetes, asthma and hypertension. We provide clinicians with recommended procedures, monitor the patient treatment process, and track/report on a number of health status indicators and outcome measures. The technology used to do this contains many of the components of CatalystTM and will eventually be wholly integrated into the CatalystTM model.
